A State of Illinois Certificate of Child Health Examination form must be completed BEFORE your student can begin an EARLY-LEARNING PROGRAM, KINDERGARTEN, SIXTH, OR NINTH grade. Your healthcare provider will provide you with a completed copy of this form to turn in to the school.
If this form is not on file at the school BEFORE THE FIRST DAY, your child will not be allowed to attend class. We highly recommend that you turn in the completed form as soon as possible NOW or at registration, to avoid BOTH YOU AND YOUR CHILD waiting in line on the first day of school to have the physical and immunizations reviewed by the nurse.
Please ensure ALL sections of the physical exam form are filled out completely. It will be accepted by the school only when all three sections are completed. Also, you should keep a copy of the physical for your own records.
- IMMUNIZATIONS – to be completed by a health-care provider. When any immunization is given, it must be verified by a signature and dated ONLY by a doctor, nurse or other health-care provider.
- HEALTH HISTORY – to be completed by the parent or guardian, signed and dated.
- PHYSICAL EXAMINATION – The examination must be performed by a physician licensed to practice medicine in all of its branches, an advanced practice nurse who is authorized to perform health examinations, or a physician’s assistant who has been delegated the performance of health examinations by their supervising physician. The physical examination may be completed any time within one calendar year prior to the first day of school. Athletic physicals are valid for 13 months from the date of the exam, and may NOT be substituted for school physicals.
Please also be aware that a DENTAL EXAM is required by the State of Illinois for those students entering KINDERGARTEN, SECOND, SIXTH and NINTH grades. And lastly, a VISION EXAM at an eye doctor’s office is required by the State of Illinois for those students entering KINDERGARTEN or an Illinois school for the first time. Your child will NOT be excluded from school if the dental or vision form is not turned in.
